Responsibilities

• Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care
• Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
• Manage the care plan throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members

Requirements

• Active, unrestricted Registered Nurse (RN) Texas License or Licensed Professional Counselor (LPC) or Licensed Bachelor Social Worker (LBSW) or Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
• 2+ years of experience working with the behavioral health setting
• Intermediate level of proficiency working with reputed company, reputed company and reputed company Outlook
• Reliable transportation and the ability to travel up to 75% of the time in the Sugarland or Meadows reputed company, TX this assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
• Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
