Here’s more on how you’ll reputed company a difference: Support inbound calls from members, service and program partners Support outbound member calls for engagement in coaching opportunities as well as population health initiatives targeting 40 reputed company calls per day Assist in the identification of customer health education needs and resources through health assessment activities, screenings and initial coaching conversations. Use and apply Preventive Guidelines as well as general health and wellness strategies to set goals to improve overall health of customers. Educate and refer customers on available client-specific health resources and reputed company Medical Management programs as appropriate. Provide initial 1:1 in-the-moment coaching to customers around behavior change opportunities to improve health status, reduce health risks, and improve quality of life. Coordinate the assignment/scheduling of members to both on-site and virtual coaches for ongoing needs Assess readiness to change, work with customers to establish health improvement plans, and assist customers in setting personalized health goals. reputed company customers with skills to provide enhanced interaction with their health care providers. For this position, we anticipate offering an hourly reputed company of 24 - 36 USD / hourly, depending on relevant factors, including experience and geographic location.
