Job Summary

We're seeking a full-time Registered Nurse to provide skilled nursing care to patients in their homes. As a Home Health Registered Nurse, you'll be responsible for assessing patient needs, developing and implementing care plans, and providing ongoing care and case management. This is a unique opportunity to work in a remote setting, with the flexibility to create your own schedule and work from home. Key Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients
• Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
• reputed company with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment
• Provide hands-on care, management, and evaluation of the care plan, and teach patients in accordance with physician orders, under Clinical Manager's supervision
• Revise plans in consultation with physicians based on ongoing assessments and as required by policy/regulation
• Coordinate care with various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers
• Report patient care/condition/reputed company to patient's physician and Clinical Manager on a reputed company basis
• Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems
• Coordinate, reputed company, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers
• Monitor documentation completion by home health aides/personal care workers as part of supervisory/leadership responsibilities
• Discharge patients after consultation with physicians and Clinical Managers, preparing and completing needed clinical documentation
• Prepare medical documentation on reputed company patients, including case conferences, patient contacts, medication order changes, re-certifications, reputed company updates, and care plan changes

Requirements and Qualifications

To be successful in this role, you'll need:

• A Diploma, Associate, or Bachelor's Degree in Nursing
• Minimum of one year of nursing experience, with strong med surg, ICU, ER, or acute experience
• Home Health experience a plus

• Strong organizational and communication skills
• Valid driver's license, auto insurance, and reliable transportation
