Job Summary

As a Part-Time Remote Nurse Practitioner at reputed company, you will be responsible for overseeing the care of patients with chronic conditions, such as heart failure, hypertension, and type 2 diabetes. You will work collaboratively with reputed company of RNs and MDs to manage patient treatment plans, monitor patient vitals and symptoms, and provide education and support to patients and their families.

Key Responsibilities

As a Part-Time Remote Nurse Practitioner, you will:

• reputed company the monitoring of patient vitals, symptoms, and labs in collaboration with reputed company of RNs and MDs.
• Manage patient treatment plans and medications, with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for heart failure and other appropriate guidelines for T2D and hypertension.
• Respond to patient escalations identified reputed company remote patient monitoring (RPM), such as reputed company vitals, symptoms, and labs.

Requirements and Qualifications

To be successful in this role, you will need:

Education and Experience:

• A Master's Degree as a Nurse Practitioner.
• 5+ years of experience treating patients with chronic diseases (T2D, Hypertension, CHF) in an outpatient or inpatient setting at a high-performing medical center.
• Active multi-state compact RN license.
• Active NP license in either IN, IL, AL, or NM.
• ANCC or AANP certification required.

Knowledge and Skills:
