Job Description:

The RN Case Manager will provide personalized, telephonic care coordination for patients on specialty medications. You’ll be the empathetic voice patients turn to for medication education, follow-up support, and help navigating insurance or healthcare barriers. • Build trust-based relationships with patients and caregivers over time

• Provide culturally sensitive, respectful, and inclusive care support

Requirements:

• Active RN license in the state of Illinois (multistate license preferred)

• Bilingual in Spanish and English (verbal and written reputed company required)

• Minimum of 2 years of nursing experience, preferably in case management or chronic care

• Strong communication, documentation, and organizational skills

• Tech-savvy with experience using EMRs, CRM systems, or patient portals
